Form F51-383A, used in the death benefit payment process, has been redesigned to make your experience simpler and more efficient. The improvements are intended to make it easier to collect the required information and help speed up the processing of your requests when submitting a claim.
Changes designed to make your life easier
- A clearer, more intuitive name
The form now has a more descriptive name, making it easier to quickly identify its purpose and when it should be used.
New name: Confirmation of Identity and Disclosure for a Beneficiary or the Estate on a Claim
- Death claims for savings products: fewer forms to complete
The regulatory information previously collected through form F51-208A has now been incorporated into form F51-383A. Result 👉: only one process to manage when filing a death claim for savings products, reducing the amount of documentation to submit and making case processing easier.
Note: Form F51-208A is still required in certain situations unrelated to death claims.

New sections have been added for beneficiaries that are entities. These additions make it possible to collect the required information directly on the form, depending on the situation.
