Many clients and advisors have brought up issues with the F5A form (too crowded, sections too short to be filled in by hand, lack of sections for multiple designations, confusing signature section, etc.).As of today, the F5A form will be divided into three separate forms:

The new forms are now available in French and English in the Documentation Centre in your Advisor Centre.
Thanks to e-signature, the EVO electronic change tool is still the easiest and quickest way to have changes made without having to submit paper forms.
